Thank you very much for your participation in the Independent Schools' benefits survey! We are excited about the opportunity to provide you with the information you need to enhance your knowledge of benefits offered in your local market. The survey should take anywhere from 30 to 60 minutes to complete depending upon the number and complexity of your benefit plans. The information required to complete the survey includes:

School Information (employees, revenue, salary, annual medical, retirement, and other benefit costs)
Per Employee Per Month Medical Premiums by plan (employee share and total employer and employee cost - for both current and prior year)
Enrollment by Plan
Plan Design Information (deductible, coinsurance, copays, HRA, HSA, lifetime max, pharmacy, etc.)
