Dear Survey Participant,

Thank you very much for your interest and participation in the Navigator Benefit Solutions ACO survey! We are excited about the opportunity to help you understand what your local health care colleagues know about and how they are responding to the ACO opportunity.

Under the Patient Protection and Affordable Care Act, providers who establish Accountable Care Organizations have the opportunity to share in savings when the cost of health care for their Medicare patients is below that of a similarly situated benchmark population. This survey seeks to understand the provider community's knowledge of and interest in ACOs. The survey is fairly short and should not take any more than about 10-15 minutes to complete. It is an opinion survey, yet also offers some insightful new information for you to consider.

If we receive a credible sample of responses, we intend to provide you with a summary, on an anonymous basis, of how you and your colleagues view the ACO opportunity. You may complete as much or as little as you like, yet the more you complete the more we will be able to share.
